Apple has released the watchOS 5.2.1 update, which now includes the ECG app. It can monitor heart

To perform an ECG, the application useselectrodes in the Digital Crown and rear panel. When you first launch the application, it warns that it is not able to detect heart attacks, but it can potentially detect symptoms of atrial fibrillation and some other abnormalities. Data from the ECG application is synchronized with the Health application on iPhone with iOS 12.1.1 firmware. Important: the application is not compatible with the beta version of iOS, even if you have already downloaded the latest update.
It is unclear why the developers have introduced an age restriction. It is easy to bypass it - just change your date of birth in the Health app - but we do not recommend you do this.

Anyway, the ECG app is available onApple Watch Series 4 with watchOS 5.1.2, paired with an iPhone 5s or newer with iOS 12.1.1 on board. So far, this feature only works on the American version of the watch - but it does not depend on the region set on the smartphone and watch, so on the American Apple Watch 4, the application will launch even from Russian accounts without any additional measures.
Note that the ECG readings obtained byway, require further consultation with a specialist. In the Health app on iOS, you can easily save the results in PDF to show them to your doctor.
