The report must be filed with the FAA no later thana week before the start. SpaceX did not provide an analysis before the Falcon 9 launch. The incident occurred on August 19, 2022. After an investigation, the company was fined $175,000. The maximum fine in such situations is $262,666. SpaceX does not comment on the story.
